FILE OF LIFE

The Carlisle Firefighter’s Relief Association working in conjunction with the Carlisle Council on Aging has purchased “FILE OF LIFE” kits for each Carlisle senior.  The “FILE OF LIFE” contains a mini medical history posted on the outside of one’s refrigerator.  In an emergency the “FILE OF LIFE” card enables emergency medical technicians (EMT’s) to obtain a quick medical history and vital personal information when a patient is unable to offer it.

 The card, which is kept in a red plastic pocket labeled “File of Life”,  lists the patient’s name, emergency contacts, insurance policy and social security numbers, health conditions, medications including dosages, allergies, recent surgeries, religion, doctor’s name and whether or not there is a health care proxy or living will.  The entire pocket is held, with a magnet, to the outside of the refrigerator for easy access in an emergency.

 Working with the Council on Aging the Carlisle Firefighter’s Relief Association is providing these “ File of Life “ pockets to Carlisle seniors at no costs as a community service project.

 The  “ FILE OF LIFE “ will be available at the Council on Aging Booth on Old Home Day as well as the Carlisle Council on Aging office at the Town hall.  If you are unable to pick one up or have any questions about the “ File of Life “ program, please call 978-371-2895.
